A downloadable form for the Arizona unemployment application can be found at https://egov.azdes.gov/cmsinternet/main.aspx?menu=234&id=2636.  These forms are provided in downloadable form because of problems with the online forms and the increase in unemployment applications.  The Arizona unemployment application has three parts that must be filled out and mailed in or faxed to the Department of Economic Security.  Once your application is processed, you will be sent information on additional instructions and requirements.

 

The first part of the Arizona unemployment application is the Arizona initial claim for unemployment insurance.  This form can be filled in online and printed out, or you can print it out and fill it in by hand.  This form asks the usual questions such as name, date of birth, social security, citizenship status, etc.  It is a two page form that needs to be filled out completely and signed by you.

The second part of the Arizona unemployment application is the Eligibility Review Questionnaire.  On this form, you must answer questions such as what kind of work you were doing when you lost your job, how much money you were earning at the time, what kind of job you are willing to accept, and the lowest wage you are willing to accept.  You also must indicate what days and shifts you are available for work, as well as how many miles you are willing to commute for work.

 

The final part of the Arizona unemployment application is the Work History form.  This form, as it states, will ask you to list all your work history for the past two years.  This form is used to determine the kind of unemployment benefits you should apply for.

 

Submitting your Arizona unemployment application does not begin the weekly claim process.  You must submit weekly claim benefits in order to remain eligible for Arizona unemployment compensation.
